For more than 45 years, HRSA-supported health centers have provided comprehensive, culturally competent, quality primary health care services to medically underserved communities and vulnerable populations.  Health centers are community-based and patient-directed organizations that serve populations with limited access to health care.  Health Centers champion preventive care and advance the medical/health home model of coordinated, comprehensive, and patient-centered care, coordinating a wide range of medical, dental, behavioral, and social services.

 

Did You Know?

  • Health Centers cared for 21.1 million patients last year.
  • About 4 million of these patients received dental services and 1 million received mental/behavioral health care.
  • About 36 percent of health center patients have no health insurance.
  • Health Centers provide primary care, defined by the Institute of Medicine as: The provision of integrated, accessible health care services by clinicians who are accountable for addressing a large majority of personal health care needs, developing a sustained partnership with patients, and practicing in the context of family and community

At OACHC our aim is to provide technical assistance (TA) to all FQHC and FQHCLA locations throughout Ohio. We are here to help support and champion quality health care and improved outcomes for the patients we serve.

 

We have a Board driven Quality Committee that meets monthly via conference calls and face to face on occasion. We provide two conferences per year and there are always quality/clinical related sessions for you to attend. In addition, we have the ability to provide various webinars and /or conference calls around topics that are meaningful to you. An example is the Ohio Learning Community (OLC) Quality Improvement Leadership Development training series, focusing on Operations and Quality personnel. We have also an archieved Medical Home Webinar Series, focusing on the journey to practice transformations and NCQA recognition. We will continue to rollout additional areas of TA through the OLC.

 

 

OACHC produces a bi-monthly quality newsletter, the Clinicians’ Corner; which highlights activities going on in our centers around patient care, patient centered medical home activities, things or people you need to know and more!  Send us your information about activities you are doing in your health centers, or better yet… submit an article about your activities!

 

We also have a clinical list serv to help facilitate communication from us to you and you to us! Please contact Ashley to be placed on the clinical list serv. You will receive our newsletters and other timely clinical information from us; use it to ask a question to another center, share ideas, problems, etc., via the list serv.

Stay Connected!

Did you know OACHC has a clinical list serv to communicate with all interested health centers clinicians? This list serv is a two way communication for you to post best practices, or ask questions that you may need assistance with from your health center peers.  OACHC also sends out important clinical  information/updates on a regular basis.
