Shannon Nielson, MHA, PCMH-CCE
Chief Operation Officer, Centerprise, Inc.

 

As Chief Operating Officer, Shannon Nielson leads the Centerprise Inc. team of expert consultants. A proven strategist and hands-on change agent, she guides provider practices in the transformation journey to successful value-based care. She is a veteran leader in primary care operations, with more than 14 year of experience in developing and implementing population health strategies including Patient Centered Medical Home (PCMH) transformation, ACO establishment, Meaningful Use achievement, EMR optimization, organizational behavior and practice management.

Today, Shannon is a sought-after speaker and resource to national and state associations and suppliers that support Community Health Centers (CHC), including the National Association of Community Health Centers (NACHC) and the National Committee for Quality Assurance (NCQA). In-person and via webinars, she presents to thousands of people annually. She authored the National Association of Community Health Centers’ Population Health Guidebook, a transformation readiness guide, and serves as Lead Faculty for the national Population Health Collaborative.

Shannon joined Centerprise in 2014, first establishing and providing services to clients through the new consulting division, and then serving as interim Chief Executive Officer. She also developed best-practice tools and templates to position primary care practices for sustainable success.

Shannon came to Centerprise from HealthSpan Solutions. As Manager, Transformation and Compliance, she successfully led more than 75 primary care practices to NCQA Level 3 PCMH recognition and helped practices to achieve Meaningful Use attestation, valued at $500,000.  Before joining HealthSpan Solutions, Shannon served as Director of Decision Support for Healthsource of Ohio, the state’s largest Federally Qualified Health Center. During her tenure she provided the 16-site organization with data analysis to guide operational and clinical strategy and directed the PCMH transformation initiative.

Shannon received her undergraduate degree in Health Services from Georgetown University, Washington DC, and her Masters of Health Administration degree from Xavier University, Cincinnati. She is an NCQA PCMH Certified Content Expert.
