EARLY-BIRD | Available until August 23, 2019, forms post marked subsequently are required to pay the regular conference rate. 
GROUP | Group discounts are applied for the second + attendee registration.  All registrations must be processed at the same time to be eligible. 
MEMBER | Members must be in good standing with OACHC to be eligible for member discounts. 


All Cancellations must be received on or before 9/11/19. Cancellations received after 9/11/19 are not refundable. Cancellations must be submitted in writing via email to events@ohiochc.org. Substitutions are encouraged. “No Shows” are non-refundable. Event registrations are non-transferable to other OACHC or outside events. In the event that there is a "no show" with an outstanding balance, attendee will be unable to register/attend future OACHC events until outstanding balance has been paid.
