City of Cincinnati Primary Care Centers Receive Highest Level of Recognition for PCMH Model

CITY OF CINCINNATI PRIMARY CARE CENTERS RECEIVE HIGHEST LEVEL OF RECOGNITION FOR PATIENT CENTERED MEDICAL HOME MODEL
Cincinnati— Cincinnati Health Department’s (CHD) Primary Care Centers received the highest level of recognition for patient care by the National Committee for Quality Assurance (NCQA). CHD’s Patient Centered Medical Home (PCMH) model is a proactive approach to primary care that transforms the organization and delivery of health care services.
“Our health centers have consistently met national patient care and health outcome benchmarks over the past decade,” said Dr. Noble Maseru, Cincinnati Health Commissioner. He continued, “The PCMH Level 3 accreditation elevates our standard of care and acceptability by the federal government, hospitals, health systems and health care industry overall, enabling us to be eligible for a myriad of resource, funding and revenue enhancements.”
The PCMH model fosters a team approach to care coordination and whole person care using evidence-based guidelines. Medical homes can lead to higher quality and lower costs, and can improve the care experience for both the patient and provider. Each of the five Primary Care Health Centers has been recognized as a Level 3 PCMH.
“This recognition exemplifies the hard work and commitment our Health Department staff exhibits in caring for the citizens of Cincinnati,” stated Dr. Lawrence Holditch, CHD Medical Director. The NCQA seal is a reliable indicator that an organization is well-managed and delivers high quality care and service. Organizations recognized by NCQA must pass a rigorous, comprehensive review and annually report on their performance.

NCQA developed the PCMH recognition program in collaboration with four key medical professional societies - the American Academy of Pediatics (AAP), the American College of Physicians (ACP), the American Osteopathic Association (AOA) and the American Academy of Family Physicians (AAFP).
