FCC on-line portal not yet functional, so applications can be submitted via pdf and email

The FCC hasn’t gotten the on-line portal functioning yet, but since it promised to start accepting applications at noon ET today, they have provides a pdf form that applicants can fill out instead and submit by email.  (Don’t forget your supporting documentation!)  Full info is available at https://www.fcc.gov/covid-19-telehealth-program and here are the high points, including links to the form and info on how to submit it.  
 
 

Submit an Application

  1. Download a fillable PDF form for the COVID-19 Telehealth Program Application.
  2. Fill in the entries and answer the questions on the form.
  3. Save the form with the following filename template: FRN_ApplicantName_MMDDYYYY (date application submitted).
  4. Email the completed form and supporting documentation to TelehealthApplicationSupport@fcc.gov.
